grep -i IPC *

Cause: A misconfiguration or installation error occurred.
Action: Install the IPC library for this release of Oracle.
Were any OS updates done – Yes:
Oracle recommends relinking after Operating system has been patched.

Do a relink of the oracle libraries :

$ cd $ORACLE_HOME/rdbms/lib
$ make -f ins_rdbms.mk rac_on ioracle

Startup works:

[oracle-alinafe.qa.brivo.net:oracle:OmsDB2:$db1]: srvctl status database -d omsdb

Instance OmsDB1 is running on node oracle-alinafe00

Instance OmsDB2 is running on node oracle-alinafe01
